02 · When they stall
The reminder names the form they left.
Most patients start the forms and stop somewhere in the middle, usually at the one that asks about medication. A reminder that says you have forms to complete is easy to ignore. One that says the medical history is still outstanding is a specific, small, finishable task, and the Hub chases with the second kind.

03 · In the room
By the time they arrive, the chart already knows.
This is the part that separates a forms tool from a clinical system. Consent status is not sitting in another application waiting for someone to check it. The chart opens on the checklist, with each consent, its signing date and the date it lapses, so an unsigned form is a blocker the injector sees rather than one they are trusted to remember.

04 · The library
Fifty-six forms, already written.
Most clinics start by trying to convert whatever their last software used, which is why the forms project never finishes. The library ships with fifty-six, grouped by treatment: botox, chemical peel, ablative laser, brow lamination, hormone therapy with separate forms for men and women. Each carries its own validity period and version, so the Hub knows when a signature has aged out rather than treating a consent from last year as current.

05 · Afterwards
A consent is evidence only if you can prove it.
Every signature is stored with who signed it, when, and whether they signed on the link or in the room. That is the difference between having consent and being able to show you had it. It stays on the patient record with the chart it belongs to, rather than in a separate folder someone has to go and find.

Do patients get one link or one per form?
One. The Hub works out what that patient still owes you and puts all of it behind a single link, so they open one thing and finish in one sitting. Forms they have already signed are not shown to them again.
What happens if they only fill in half?
They get reminded, and the reminder names the form they left rather than telling them they have forms outstanding. A specific, small task gets finished; a vague one gets ignored.
Does the clinician know what is signed before treatment?
Yes, and that is the point. The chart opens on the consent checklist, showing each form, when it was signed and when it lapses. An unsigned form is visible before treatment starts rather than discovered afterwards.
Do we have to write the forms ourselves?
No. The library ships with fifty-six, grouped by treatment, covering botox, chemical peels, ablative laser, brow lamination, hormone therapy and more. You can create your own where the library does not cover something you offer.
Does it know when a consent has expired?
Yes. Each form carries its own validity period, a year by default, and its own version. A consent signed thirteen months ago is not treated as current, so a repeat patient is asked again rather than passing on a stale signature.
Where do signed forms end up?
On the patient record, with the chart they belong to. Every submission is stored with the patient, the form, the time it was signed and whether it was signed on the link or in the room.
Can forms go out before the appointment?
Yes, that is the normal case. The link goes out ahead of the visit so the paperwork is done before the patient is in the chair, and anything still outstanding on the day is visible to whoever opens the chart.
